‘Getting My Health Information Ready for Hospital’

WE’RE READY TO LAUNCH!

The My Health My Communication project is excited to launch the toolkit for ‘Getting My Health Information Ready for Hospital’, as part of celebrations for International Day of People with Disability. #IDPwD

The project is pleased to gain the support from NSW Premier, The Hon. Dominic Perrottet MP, and he has kindly shared a video message.

It has been a shared effort getting to this stage with contributions from people with disability, carers, support workers and hospital staff, who have allowed the project to gather essential data to identify key issues and possible solutions to address these.

These communication tools have been developed for people with disability to use to share their important healthcare information when going to hospital.
